Every vent/return in your house is at the end of a leg. This leg is connected to your main duct. In order to clean this leg we take off the vent cover and send a long hose in to the vent opening and through the length of the leg, using compressed air together with various cleaning nozzles to usher the debris all the way through the leg to the main duct .
